What to Bring

- List of current medications
- Medical history
- Copy of medical records, if accessible
What to Expect
Please allow at least an hour for your first visit. We will go through your medical history together and show you how to use our patient portal. Completing your BFM forms beforehand helps reduce your wait time.
Please note: we are not a pain management clinic
If you are currently managed on controlled substances, we will require an initial consultation to determine whether this treatment aligns with our healthcare approach before continuing care.
